WHO WE ARE

Along In Years, LLC is the brainchild of A. Lisa Raynor, DNP-C, MBA, MHA, BSN, RN.

Ms. Raynor has 29 years of nursing experience, with 18 years in community health, serving in various roles, including Director of Nursing, Clinical Nurse Specialist, Senior Project Manager, Senior Director of Clinical Operations, and Vice President of Home and Community-Based Services.

Recognizing gaps in care services within current healthcare systems, Along In Years, LLC (AIY, LLC) was created.

AIY, LLC, developed InGap Care™ (IGC), a structured consulting and support framework. It’s designed to ensure that what happens at home aligns with preventing avoidable setbacks, improving care transitions, and easing care burdens on families.

At the heart of AIY, LLC’s Mission is a commitment to providing client-centered health management and monitoring through wrap-around care that guides participants and families.

Our Vision is not just to provide care, but to be the bridge that closes the noted gaps in healthcare.
